Gems are not very rare items in Diablo 4 and can be found across Sanctuary’s open areas. Players can slot them into sockets found on armor, weapons, or jewelry to hone their build even further. Although gems provide significant buffs, gamers need to upgrade the item to get the most benefit.
Interestingly, crude crystals are the basic variant of gems and skulls are fairly common drops from ore veins that are scattered through the map. There are a total of seven different types of gems in Diablo 4:

How to upgrade Gems?
After reaching level 20, players can upgrade gems at any jeweler. Once the fans complete the Gem Crafting quest and talk with the jeweler Kratia, who can be found in Kyovashad, the gem upgrade feature gets unlocked. Later, they can talk to any jeweler across the Sanctuary and upgrade their gems.

Furthermore, players can also combine at least three crude crystals or skulls of the same type and fork over some coins to create gems themselves. Once they reach level 40, the feature to create gems gets unlocked. Moreover, Levels 60 and 70 grant you Flawless and Royal gems, respectively; thus, three Flawless gems can be converted into one Royal gem, and so on.
