Regilaneo da Silva In?cio was performing squats on a squat machine when the less than 60-day-old machine, malfunctioned. While free-weight barbell squats are arguably among the best exercises one can do, squat machines have several use cases. Besides providing greater isolation of the quads, machines are also easy on the joints and ligaments. Machines also allow people to lift heavier.

The 42-year-old was performing squats with 300 pounds on the machine. In the CCTV footage that captured the incident, Silva finished his set, engaged the safety lever, and climbed down. However, instead of resting on a bench or something, he sat on the machine platform. Unbeknownst to him, the safety lever malfunctioned, and 300 pounds came crashing down on his neck.
